Policy for Management of Personal Information

This document describes the policy of Dr Georgina O’Donnell for the management of client information. The psychological services provided are bound by the legal requirements of the Privacy Act 1988, including the Australian Privacy Principles, and the Freedom of Information Act 1982.

Client information

Client files are held securely by Dr O’Donnell. The information on each file includes personal information such as name, address, contact phone numbers, legal documents, and other information that is relevant to the psychological services being provided.

Purpose of holding information

The information is gathered as part of the assessment, diagnosis and treatment of the client. The information is retained in order to enable Dr O’Donnell to provide a relevant and informed psychological service, and as a reference in Court proceedings where relevant.

Requests for access to client information

At any stage clients may request to see the information about them kept on file. Dr O’Donnell may discuss the contents with the requesting client and provide a copy if needed. All requests by clients for access to information held about them should be lodged with Dr Georgina O’Donnell. These requests will be responded to within 14 days and a consultation appointment may be required to discuss the file information.

Any request by a third party to access file information will be discussed and approved by the client, except in the case of a legal subpoena in which file release may be legally mandated.

Website

This website is securely hosted in Australia. When you visit this website basic traffic information is logged which includes your IP address, date and time, type of web browser, and pages accessed. Logs are only processed for security and reliability purposes, are stored in a secure environment, and are automatically deleted within 30 days.

Other than the basic traffic logs mentioned above, this website does not use cookies, analytics, advertising, third-party social media buttons, or any other mechanism designed to track your behaviour.
